Real or Fake?

Cardiosurgical cycle is going on now. Not a heavy cycle but rather a time wasting somehow. We have to wait for unknown reasons, however it's good in a way that we can talk and gossip, and laugh like nobody's business.

The 1st few days were okay because English speaking teachers delivered the classes smoothly. The first one was a happy lady, 2nd one was a fast speaking young lad, and the 3rd one was a cute, very young good-looking doctor. Today's the 4th day, the Bear man (Medvedev, a bloke) was the teacher, and the class was kinda boring and russian-ish.

He knows some english equivalent terms but not all, which we found difficult in understanding. Hence he taught us with drawing......drawing......and more drawing. And we answered with our mediocre russian level and more drawing too. We managed to understand, if not all, the things he tried to explain pretty well.

But one thing's for sure, he smokes and he has a bad mouth odour. Ergh I couldn't stand sitting right beside him while he was explaining to us. It was like either a bad meat or a good cheese, you call it. Due to long standing babbles of his in Russian, we were kinda blew away by him.....as in to the slumberland.

"So tell me what happened if.............." (in Russian)
"zZzZzZzZz.................."
"Understand? Any questions?" (in Russian)
"Aaaaa........ha........Mmmmm......hm"

He played 3 video clips about valvular replacements. It was interesting but the volume of the video was so muffled. It was like a lullaby, sending us to sleep. We all managed to go anti-sleep but Nantha surrendered and dozed off in front of the Bear on a soft comfy couch.

So the class was devoted to Valvular Heart Diseases. He showed us some dried, well perserved heart valves with calcifications. It's somewhat astonishing and amazing because we would never (or have yet) seen calcified valves in real.

So what would you choose?

A real flower or a fake one?
A real Bush or a fake one?
A real dick or a fake dildo?
A real valve or a prosthesis?

Of course both have their own benefits. When you're out of choice, a fake one would come in handy. But there's nothing more satisfying than having an original one.

A real flower may dry up, but a fake one stays forever.
A real Bush can be seen on tv, but a fake one can be found only in Wax Museum.
A real dick.........well you know~don't need to go further than that.

Of course, a real valve is yours, a prosthesis is a replacement. Nothing is better than an original but when there's no choice but to replace it, a prosthesis is the answer.

So reasons causing the calcification of the heart valves and stenosis(narrowing) are amongst the following: -
a) Rheumatism - tonsillitis, joint pain etc
b) Infection - infective endocarditis
c) Trauma of the valve
d) Atherosclerosis - especially in aorta, the plague restricts opening of aortic valve.
e) Degeneration of valvular cusps
f) etc...........

Read ShouZhi's blog in both chinese and english. He wrote it interesting.

Ooooookay.....getting boring here, take a look at the pictures. I'm not gonna tell here more for your understanding. Any questions, feel free to PM me.

Now you see..the yellowish is the valvular structure, the whitish is calcification.

The prostheses of valves.

The forefather of all prosthetic valves.

The newer prosthetic valve, the aortic prosthesis, and calcified valves.

The size of a valve in comparison with ShouZhi's dry cracked palm.

The latest modern prosthetic valve and its old friends.
Up close

Now, altogether, say cheese!

Patients with prosthetic valves have little cute ticking sounds coming out from the chest. Kinda freaky though.

I’ve been following your blog and some other pages of your friends for some time now and its sad that nothing positive is ever written about our НижГМА or its academic staff. Like many other entries complaining of delays, how disappointing things are when you go to class with bad English and all Etc it has been a vicious cycle of complaints cycle after cycle. What I am trying to say(My point) is that blogs are being shared by the same group of colleagues and they don’t need to know what’s going over at these classes because the scenario is almost the same with each cycle from the day one at the academy except for very few classes and cycles. But for others visitors like your relatives and friends viewing your blog who has no idea about NNSMA might get the wrong impression about the place where you are getting your higher education. For example when I was reading your above entry I felt the same way back then about Medvedev. At that time, when we had classes at Cardiac center and like you said Medvedev was dead boring. All he did was to explain what a bypass draft is (the mammary and the coronary) and the difference between pressure gradients of heart chambers and those videos you’ve mentioned. The prosthetics and valves were also there. We were not demonstrated even a live surgery! So true cardiac surgery seemed so dull. Then we were not given credit ”Sachauth” on time since the whole group was absent on certain days missing it purposely because it was so boring. Then I was asked to meet him at 5th Hospital after a week to get the signature on our credit books. So I collected all credit books and met him at his office. I was impressed; everybody knows Medvedev is a top cardiac surgeon in Russia but nobody knows to what extent he’s popular. Among dozens of certificates and honors hanging over his desk at his office, there was one saying that he has been bestowed a honorary citizenship of Europe. He was a fucking honorary citizen of whole of Europe. See… like we complain about most of our teachers but they are quite recognized and potent and charged with many things beyond our knowledge. Proff Pichugin, Parshikov, Zigmentovich none of them speak good English and we used to complain a lot. But they are quite renowned with academics around the world. So like I said why always give the bad impression about our “Alma mater”?!

PS: Sorry this comment had to be longer than your post..lol

Pichugin's and Ziggy's are good to me, at least they speak english rather than not. I never complained about how bad a russian can speak english. What matters to me is the degree of understanding I can get from them.

Scenario of each cycle isn't the same because we do different stuff during different cycles, and my readers enjoy reading. I do not wish everyone to agree with me, but im certainly doing what i wanna do.

People complain everytime, we can't say anything to change everything. You used to complain the same way back then from what I understood . It's just a matter of now or never.

Don't tell me you've never complained about each cycle like Hygiene, Epidemiology, Public Health etc.

Even the exchange students from Austria mentioned about something stupid happening during their exchange in this academy.

But seriously, the negatives are more than the positives in this academy.

I'm impressed when you said that he was granted a citizenship of European Union. I bet he knows Dutch because all the video clips he showed to us were in Dutch.

In my humble opinion, our Alma Mater is somehow better than some other Med U in Russia.

Yes it is an honor to have such renowned teachers in medical field, but i don't see how I can praise our "Alma Mater" with their great achievements. With such an awful system of handling students? I don't think so....

And since the issue which the academy wanted us to pay 500USD for the extra practical (you know better about practical here), I have got nothing good to say but to blame them to make us pay so much.

PS : Thanks for the long comment and follow up. This is the longest comment I've ever have. Stay tuned for more.....hahaha

500$ for some extra piece of shit is too much and I think, this you should strongly protest. But what’s 500$s for you guys it’s just a months allowance! Pay and staying at home is the other alternative. Somehow what you say is true less positives and more negatives at NNSMA. But thinking positively can make lot of differences (the power of positive thinking) Anyway I don’t know what’s wrong with people over there (dekanath)….may be you’re paying a lot for the little that they are giving you. They need good lessons and some workshop on maintaining international standards in any case certain things we can’t change. Things can get bit frustrating sometimes but I hope you all will hang on to things..! Tc

Well you are not in our shoes, so you don't know the ordeal we've to go through. 500USD is not monthly allowance to all, so you can't imply this matter to all Malaysians. Some only have 300USD to get a life here. Some lesser. Those Malaysians that you knew were probably the richer ones.

Yes by thinking positive is probably the ONLY option we have to relieve the stress we're having now. But sometimes things gone overboard and there's only mental turbulence. And hence, complaints like so you're reading now.

Dekanat is always making us frustrated in so many ways. So to sabotage, this is what we do. We write, we blog, we publish to all. So mind me, if you didn't like what you read. We've done giving feedbacks to the Dekanat, and nothing seemed to change. So what I'm doing now is sort of a payback, if succeeded.

Somehow this act is childish but yet fun and exciting and I like to do it very much.
